When clients book for therapy sessions I note down their personal data and contact details, including name, address, DOB, mobile number and e-mail address.
I may also record GP information, for safeguarding reasons or if there is a concern over client safety or the safety of another.
During client sessions I take handwritten notes, which are stored in a lockable cabinet in my office. This cabinet is locked at all times when not in use, where my session notes are stored securely and offline.
Personal data is stored until 7 years after the client has completed therapy with me (in accordance with my professional insurance terms), ?at which point all paper records are securely shredded. Any electronic records or information (resulting from a client either contacting me via a form on my website or e-mailing me during our time working together) are deleted as soon as I have responded to the enquiry or e-mail.
Personal data will never be shared with a third party unless the client is at risk, is suffering harm or is a risk to others.
